What You'll Do As aPhysical Therapist Assistantin our Home Health team, you'll:
Deliver skilled physical therapy interventions in patients' homes under the plan of care set by a licensed Physical Therapist.
Educate patients and caregivers on safe mobility, home exercise programs and adaptive equipment.
Document treatments, progress notes and reassessments in our electronic health record (EHR) in real time.
Collaborate with an interdisciplinary care team—including PTs, OTs, nurses and social workers—to optimize outcomes.
Travel within our Iowa/Illinois service area using your reliable vehicle (mileage paid).
