Nationwide Therapy Group is seeking a travel nurse RN Case Management for a travel nursing job in Fort Defiance, Arizona.
Job Description & Requirements

- Serve as a primary RN case manager in a busy outpatient primary care setting.
- Collaborate closely with providers and support nursing staff to:
- Develop and manage patient care plans
- Monitor patient outcomes
- Coordinate follow-up care and resources
- Advocate for patients and assist in navigating healthcare services, especially for chronic conditions and preventative care needs.
- Work as a liaison between the patient, providers, specialists, and community resources.
- Ensure seamless coordination of care, referrals, and follow-up treatments.
- While case management is the primary function, the nurse may be floated to other departments based on:
- Clinical qualifications
- Experience in inpatient or task-oriented roles
- Examples: answering phones, assisting patients, taking light patient loads
- Strong in care planning, patient education, interdisciplinary communication
Licensure: Active RN license (valid in Arizona or compact license)
Experience: Minimum 3 years of recent case management experience required
