Requirements
- Active RN license (Nebraska or Compact)
- BLS (AHA only)
- Minimum 2 years LTC RN experience
- Clean license and background (discipline/criminal history must be disclosed)
- Resume with full 7-year work history (facility names, not agencies)
- 2 recent references (1 must be supervisory)
- Skills checklist (within 1 year)
- Immunizations: MMR, Varicella, Hep B, Tdap, Flu (seasonal), TB (or Chest X-ray + questionnaire)
- Background: National Criminal, FACIS III, Sex Offender, OIG, SAM, SSN trace
- Nebraska DHHS Registry Check (APS/CAN)
- Valid state-issued photo ID
- COVID vaccine or declination
