ft: 8H Days 0830-1700 + EOW; minimum RTO strongly preferred; first to float Description: Unit Notes:*** 3 years of performing Case Management/Care Coordination in an acute hospital setting within the last 5 years***Unit: Care Coordination (case management) Certs:MUST have experience with and proficient at Midas, InterQual, and Microsoft Office programs. Experience with discharge planning AND utilization review. Minimum of 3 years hospital experience. Must have recent experience with Interqual (within past 6 months) EMR They do not have to be proficient in Cerner or Curaspan, we can teach that. They do need to have experience working with an EHR (electronic health record)Ratios:1 to 20 patient assignment Charge Nurse:Manager Weekend Requirement:Every other weekendScrub Color:Business casual conservative dress, no open toed shoes, and lab coats.
