Job Summary
The Oncology Nurse Navigator (ONN) is responsible for:

Individualized assessment of cancer patients, family members, and caregivers across all phases of care

Identifying personal and systemic barriers to care

Problem solving and making referrals to improve access and outcomes

Collaborating with the oncology team and reporting to the Director of Oncology and Infusion Services
