Schedule: Monday - Friday with on-call rotation (back-up)
Territory: Mobile county communities

WhatYoullDo:
- Deliver personalized hospice carethat honors each patients unique needsdocument every step to ensure accuracy and continuity.
- Lead with empathy during home visits, assessing patient and family needs and crafting care strategies that prioritize comfort, dignity, and peace.
- Empower families and caregiversthrough clear education on hospice services, symptom management, and available support resources.
- Guide and mentor your team, training Hospice Aides and LPNs to deliver safe, effective, and compassionate care at home.
- Collaborate seamlesslywith physicians, interdisciplinary teams, and facility staff to create smooth transitions and coordinated care plans.
- Provide hands-onexpertisein pain control, medication administration, and symptom reliefbringing comfort when it matters most.
- Be a trusted resource after hours,participatingin on-call rotations to ensure patients and families receivetimelysupport on weekends and holidays.
- Maintain compliance and eligibilityby monitoring patient status and completing all certification and recertification documentation with precision.

What You Need:
- Active RN license in state of residence
- Current CPR certification
- Valid drivers license, current carinsuranceand reliable transportation
Preferred Qualifications:
- At least 1 year of nursing experience inhospice, home health,med surge,or ER
