Position Overview
We are seeking a dedicated and licensed Physical Therapist (PT) for a 13-week contract assignment in a Skilled Nursing Facility (SNF) / Long-Term Care (LTC) setting in Texas. The ideal candidate will have experience working with geriatric populations and be comfortable managing a diverse caseload focused on rehabilitation and functional improvement.
Key Responsibilities
- Evaluate and develop individualized treatment plans for residents
- Provide therapeutic interventions to improve mobility, strength, balance, and overall function
- Collaborate with interdisciplinary team members to ensure comprehensive patient care
- Educate patients, families, and caregivers on treatment plans and home exercise programs
- Complete timely and accurate documentation in accordance with facility and regulatory guidelines
- Ensure compliance with all state and federal regulations
Qualifications
- Active Texas Physical Therapist (PT) license (required)
- Minimum of 1 year of experience preferred, ideally in Skilled Nursing or Long-Term Care
- Strong clinical assessment and care planning skills
- Excellent communication and teamwork abilities
