Travel Registered Nurse (RN) – Case Manager
Apple Valley, CA
Duration: 13 Weeks
Shift: Days – 5x8s (8:00 AM – 4:30 PM)
Weekend Rotation Required (every other weekend)
Important Requirements
- Seasoned traveler required (HARDSTOP – no first-time travelers)
- Must have 2+ years Acute Care Case Management / Discharge Planning
- Behavioral health–only experience does NOT qualify
- Active CA RN license (pending accepted but must be active by start date)
- Epic experience required
Position Overview
Seeking an experienced Acute Care RN Case Manager to manage a caseload of up to 25 patients in a hospital setting. This role focuses on care coordination, utilization review, and complex discharge planning across diverse patient populations.
Core Responsibilities
- Patient assessment & needs evaluation
- Care coordination & discharge planning
- Utilization review & admission criteria
- Participate in treatment plan development
- Organize & lead family meetings
- Arrange DME, SNF placement & ambulance transport independently
- Prior authorizations & benefits eligibility review
- Disease management & plan of care oversight
- Regulatory compliance (CMS, DRGs, Joint Commission standards)
- Document in Epic EMR
Required Experience
✔ Acute hospital case management experience (ED, ICU, MS, Surgical Services)
✔ Utilization Review
✔ Knowledge of HMOs, IPAs
✔ Familiarity with:
- Adoption/foster care processes
- Substance abuse treatment resources
- Homeless patient discharge coordination
- Mental health crisis support
- Advance directives & POLST
- Protective services processes
