Registered Nurse (RN) – Home Health
A Home Health Registered Nurse (RN) provides skilled nursing care to patients in their homes. The RN assesses patient health, develops care plans, administers treatments, and educates patients and families to promote recovery and independence.
Responsibilities:
- Perform patient assessments and develop individualized care plans
- Administer medications, treatments, and wound care as prescribed
- Monitor patient progress and update care plans as needed
- Educate patients and family members on disease management and home care
- Maintain accurate and timely patient documentation
- Coordinate with physicians and other healthcare professionals
- Ensure compliance with home health policies and healthcare regulations
Requirements:
- Active RN license
- Experience in Home Health or Acute Care preferred
- Strong clinical assessment and communication skills
- Ability to work independently and manage multiple patients
