Job Summary

🩺 Patient Care Responsibilities
- Create and manage individualized patient care plans.
- Adjust the care plan based on how the patient responds to treatment.
- Evaluate effectiveness of treatment and outcomes.
👩⚕️ Care Coordination
- Coordinate and supervise patient care when required.
- Work with the healthcare team to ensure proper patient treatment.
📋 Organization & Prioritization
- Ability to organize workload and prioritize tasks efficiently during shifts.
📚 Assessment & Education
- Use patient assessment data to determine care needs.
- Provide patient education and discharge planning.
⚠️ Monitoring Patient Condition
- Identify abnormal findings during assessments.
- Report issues and update the care plan accordingly.
📝 Documentation
- Accurately document all patient care activities, including:
- Wound assessments
- Patient education
- Progress toward care goals
- Medication administration
- Nursing flow sheets
- Assessment forms
