You will help patients recover from injuries and manage chronic conditions while working within a home health practice and covering Delta County and portions of Dickinson and Menominee Counties within approximately a 1-hour radius of Escanaba.
Minimum Qualifications
- Doctoral, Master's, or Bachelor's degree in Physical Therapy from an accredited program approved by the APTA.
- Active Michigan Physical Therapist license required to start the assignment.
- Home health experience required.
- BLS certification from AHA or ARC may be required.
- Ability to complete physical therapy admissions, evaluations, reassessments, and discharges in a home health setting.
- Ability to create individualized plans of care and supervise/work with a PTA.
Preferred Qualifications
- Experience with HCHB (Homecare Homebase).
- Good navigation skills and ability to follow GPS directions.
- Wound care or lymphedema certification.
Assignment Details
- Shift hours: 0800 - 1600 (office hours noted as 8:00 AM - 4:30 PM; clinicians self-schedule).
- Workweek: Monday through Friday, office-hour schedule.
- Weekly hour guarantee: 32 hours.
- Productivity expectation: 30 points weekly.
- Coverage area: All of Delta County and portions of Dickinson and Menominee Counties within a 1-hour radius of Escanaba.
- Mileage reimbursed between visits from first patient to last patient.
- No on-call or weekend requirement.
- Dress code: Navy blue scrubs or navy blue polo with khaki, black, or navy slacks.

Impacting Patient Care
Provide comprehensive home health physical therapy services including admissions, add-on evaluations, reassessments, discharges, individualized plan-of-care development, and collaboration with PTAs. Participate in weekly case conferences and monthly staff meetings to support care coordination and quality outcomes.
