Job Summary
Cuready Healthcare Staffing is seeking a qualified RN Case Manager to support patient care and clinical operations within healthcare facilities. The ideal candidate will deliver safe, high-quality care while collaborating with interdisciplinary care teams and maintaining compliance with facility policies and professional standards.
Responsibilities
- Perform care coordination and discharge planning for acute care hospital patients.
- Utilize Epic charting system for accurate documentation and patient information tracking.
- Organize and participate in family meetings to discuss treatment plans.
- Arrange necessary durable medical equipment and transportation services independently.
- Coordinate with interdisciplinary teams, including social services and transportation.
- Adhere to care standards and hospital policies while managing a patient ratio of 1:20.
Qualifications
- Registered Nurse (RN) with active CA state license; pending license accepted if obtained by start date.
- Minimum of 2 years of acute care hospital case management experience.
- Experience using Epic or similar charting systems.
- Strong understanding of CMS guidelines, DRG, and InterQual criteria.
- Competent in care coordination and discharge planning for diverse patient populations.
- Travel experience required; must be willing to rotate weekends and holidays as needed.
