Mosaic Life Care seeks a compassionate Direct Care Worker to support patients with daily living activities and promote independence in a patient-centered environment. In this role, you’ll provide hands-on personal care, assist with mobility, hygiene, meals, and safety, and observe and report changes in condition to the clinical team. You’ll work closely with nurses and other caregivers to follow individualized care plans.
