Minimum Requirements
- Bachelor of Science, Master’s, or Doctoral Degree in Physical Therapy from an accredited program approved by the American Physical Therapy Association (APTA).
- Current state license as a Physical Therapist required prior to assignment start.
- Proof of current CPR certification required.
- Current driver’s license, reliable transportation, and vehicle insurance required.
- 1-2 years experience as a Physical Therapist in an acute care or rehabilitation setting required.
- Ability to perform heavy lifting, bending, pulling, pushing, prolonged standing and walking; manual dexterity and hand-eye coordination required.
- Ability to work a flexible schedule and travel locally.
- Must read, write, and speak fluent English.
- Home health experience preferred.
Preferred Qualifications
- Bachelor of Science in Physical Therapy from an APTA-approved program (preferred if not listed above).
- Previous home care experience preferred.
Assignment Details
- Candidate must be able to travel locally and provide care in patient homes.
- May be required to lift and safely handle patients and durable medical equipment in home environments.
- Works collaboratively with an interdisciplinary team and communicates with physicians as needed.

Impacting Patient Care
Provide direct patient care in home health settings by planning, implementing, and evaluating individualized plans of care. Perform clinical assessments, document progress notes and plan of care updates, instruct patients and families in safe transfer techniques and use of assistive devices, and order or adapt rehabilitative equipment to meet home limitations. Apply infection control and universal precautions. Supervise or oversee other team members when applicable. Maintain continuing education and professional competency.
