Daily responsibilities include:
See outpatients in office-based settings throughout the Hartford area, one outpatient center per day with Saint Francis Hospital as the hub.
Provide transitional care management
Offer guideline directed medical therapy optimization
Perform advanced evaluations, nuclear and stress testing.
Provide same day access care
