Position Overview
We are seeking an experienced Registered Nurse (RN) for its Long-Term Care / Skilled Nursing Facility. The ideal candidate will have strong assessment, medication administration, care planning, and critical-thinking skills and be comfortable caring for patients in a long-term care setting.
Requirements
Active Illinois RN License
Minimum 3 years of Long-Term Care RN experience
Current BLS/BCLS Certification
Graduate of an accredited nursing program
Education verification required
Long-Term Care / Skilled Nursing experience preferred
Strong assessment and critical-thinking skills
Responsibilities:
Perform comprehensive patient assessments and evaluate care needs
Develop, implement, and evaluate individualized patient care plans
Administer medications and treatments according to established standards and policies
Maintain accurate and comprehensive patient documentation
Provide patient and family education and support
Collaborate with physicians and interdisciplinary team members
Delegate appropriately and promote patient safety
Participate in quality improvement and professional development activities
Support evidence-based nursing practices and peer-review initiatives
