Minimum Requirements
- Bachelor's, Master's or Doctoral Degree in Physical Therapy from accredited schools approved by the APTA
- Active State License is Required to Start the Assignment
- BLS Certification May Be Required from AHA or ARC
- Physical Therapy licensure in state of practice: Required
- Basic Life Support (BLS OR HS-BLS OR RQI BLS) certification: Required
- Must have EPIC experience
- Pelvic floor therapy experience
- PRPC or WCS certification or working towards certification

Location Highlights
Ukiah offers access to Northern California outdoor recreation and wine country amenities. Montgomery Woods State Natural Reserve is about 30 miles west and features old-growth redwoods, while Lake Mendocino sits just north of the city for boating, hiking, and fishing. Downtown Ukiah includes the City of Ten Thousand Buddhas nearby in Talmage and the Grace Hudson Museum in the city center, giving the area both cultural landmarks and a quieter small-city setting.
