Minimum Requirements
- Bachelor's, Master's or Doctoral Degree in Physical Therapy from accredited schools approved by the APTA
- Active State License is Required to Start the Assignment
- BLS Certification May Be Required from AHA or ARC
- VA Physical Therapist license required to start.
- Physical Therapist credential verification required to start.
- New graduates are not accepted.
- Hep B required to start.
- TB required to start.
- Flu required to start.
- Tdap required to start.
- Criminal Background required to start.
- OIG Search required to start.
- Statement of Good Health required to start.
- OSHA Packet required to start.
- Verification of Professional Education required to start.
- I-9 Form required to start.
- GSA Search required to start.
- UDS - 9 Panel (Marijuana Not Included) required to start.
- Contract Therapist Compliance Attestation required to start.
Assignment Details
- Shift Hours: 8 hours.
- Patients are scheduled every 30 minutes and treated for 60 minutes.
- Evaluations are scheduled for 60 minutes.
- Productivity expectation includes 11+ visits per day, 90% productivity, and 55+ visits per week.
- Documentation system: NextGen.
- Patient population includes adolescent, adult, geriatric, and orthopedic patients.
- Staff support includes Patient Experience Coordinators and Rehab Aides.
- Dress code: Business Casual.
- Temp-to-perm opportunity available.
