Job Summary (List Format):
- Must possess and bring state or federal issued identification to the facility
- Required to wear a photo badge at all times while on site
- Must have completed the original or updated COVID-19 vaccination, or
- Provide documented religious and/or medical exemption from employer if not vaccinated
- Obtain additional COVID-19 vaccine doses if required by law
- Compliance with facility and agency policies regarding identification and health requirements
