Schedule: Monday - Friday | 8:00 AM - 5:00 PM (40 hours/week)

28 units

HH. Oasis and HCHB required

On-call required

Must have prior HH experience, OASIS and HCHB, or HCHB training before start required

Productivity: 28 units per week

Employment Type: 13-week contract

Required Qualifications

Current Registered Nurse (RN) license in the state of Colorado / Compact License
2+ years of Home Health nursing experience
Experience with OASIS documentation
Experience with Homecare Homebase (HCHB) software
Current CPR certification
Valid driver's license
Reliable transportation with active insurance
Minimum 1-2 years of clinical nursing experience

Preferred Qualifications

CHPN certification
Bachelor of Science in Nursing (BSN)

Education Requirements

Associate Degree in Nursing (ADN) or Nursing Diploma - Required
Bachelor of Science in Nursing (BSN) - Preferred

Responsibilities

Provide skilled nursing care to patients in their homes
Complete comprehensive patient assessments and develop care plans
Accurately complete and submit OASIS documentation
Document patient care using Homecare Homebase (HCHB)
Coordinate with physicians and interdisciplinary care teams
Educate patients and families on treatment plans and disease management
Participate in required on-call rotation schedule

Protouch Staffing Job ID #PROTOUCHEXT-91511. Pay package is based on 8 hour shifts and 40.0 hours per week (subject to confirmation) with tax-free stipend amount to be determined.
