Minimum Qualifications
- Active California Registered Nurse (RN) License
- Minimum of 2 years of recent Acute Care Case Management experience
- Demonstrated experience performing:
- Care Coordination
- Discharge Planning
- Utilization Review
- Resource Management
- Interdisciplinary Team Collaboration
- Previous Travel Nursing experience required (First-time travelers will not be considered)
- Strong knowledge of acute care workflows and patient care transitions.
Key Responsibilities
- Coordinate patient care throughout the hospitalization and discharge process.
- Collaborate with physicians, nurses, social workers, and ancillary departments to develop and implement individualized care plans.
- Facilitate timely and appropriate patient discharges and transitions of care.
- Assess patient needs and identify barriers to discharge.
- Perform utilization management and resource coordination activities.
- Ensure compliance with hospital policies, regulatory requirements, and quality standards.
- Advocate for patients and families to ensure optimal care outcomes.
- Maintain accurate and timely documentation within the electronic medical record system.
