Minimum Requirements
- Doctoral degree in Physical Therapy required.
- Active Oregon Physical Therapy license required to start the assignment.
- BLS certification from AHA or ARC may be required.
- Home health experience required.
- Experience with HomeCare Homebase EMR preferred.
- Ability to perform PT visits including SOC, evaluations, recertifications, ROC, and supervisory visits.
- Ability to travel within and provide coverage across Josephine County. Mileage reimbursement provided between visits from first patient to last patient.
Preferred Qualifications
- Prior home health travel experience preferred.
- Familiarity with home health productivity expectations and point systems preferred.
Assignment Details
- Shift hours: 0800-1600 (Mon-Fri schedule indicated as 800-1630 in client notes).
- Weekly hour guarantee: 40.
- Coverage area: Josephine County.
- Must perform visits including SOC, PT eval, recert, ROC, and supervisory visits.
